How should I give Laxido to my six month old?

My DS 6 months has been prescribed Laxido but with no real instructions on how to take it other than ‘put it in his milk’. I know this sounds daft but how do I do that? Do I make the laxido separately then add to his bottle or add it in at powder stage? He is formula fed.

Needs to be dissolved in water first and then added to the made up formula, it won't mix properly if put it directly into milk

Edit - ensure the water that you dissolve the powder in is preboiled
